Hormone replacement therapy when blood levels change and is not for everyone. Some of the side effectsIt has now been determined that many patients are suffering from Long Haul COVID-19 Syndrome (LHCS).symptom of hot flashes, insomnia, or mood changes occur. Perimenopause may begin in the 40s when the ovaries are beginning to decrease in function. Periods may be irregular and restorative sleep disappear due to night sweats. Progesterone declines first of the three sex hormones, estradiol, testosterone, and progesterone.Progesterone relieves insomnia, is a mild diuretic (decreases bloating), and aids in calmness. Progesterone’s great benefits have been known for more than 50 years, when a wonderful English woman doctor, Dr. Katherine Dalton, treated severe postpartum depression with high dose injections of progesterone in oil followed by suppositories of progesterone. Right after childbirth, the hormones may become unbalanced and this can cause severe mental disturbances in a few women, similar to extreme mood changes seen in some menopausal women. The synthetic progesterone, progestin, does not produce the same benefits and is an entirely different appearing molecule.Estradiol and testosterone both decline when the ovaries stop secreting them. When the brain does not receive enough estradiol, the pituitary sends out a messenger hormone, FSH, follicular stimulating hormone, to stimulate the ovary to get back to work. An FSH >23-30 is a lab value consistent with menopause.and risks of hormone replacement therapy may be an increased risk of cancer of the sex organs, blood clots, and stroke. Excess body fat, tobacco, and alcohol also have been noted as increasing cancer risk. Some of the side effects and risks of NOT taking hormones may be dementia, osteoporosis (bone loss), and general decline in energy.I have seem many women in their late 50s, 60s, and even 70s who are literally sick of the hot flashes and insomnia and finally want it relieved. I also see women whose family docs or gynecologists abruptly stop all hormones at a certain age, like 55 or 60. Debilitating hot flashes, mood and memory problems drive these sufferers to me. After careful discussion of their health history, I explain the risks and benefits of hormone replacement therapy. Some women opt to begin or continue hormones, while others do not.Hormone replacement therapy is a lifestyle choice.
